Summary

Drug-eluting stents (DES) release sirolimus to inhibit vascular smooth muscle cell proliferation, effectively reducing in-stent restenosis (ISR). Sirolimus-eluting stents demonstrate safety and efficacy in preventing restenosis in coronary artery lesions.

Area of Science:

  • Cardiovascular Medicine
  • Interventional Cardiology
  • Pharmacology

Background:

  • Coronary artery stenting is limited by in-stent restenosis (ISR).
  • Previous efforts to prevent ISR have largely been ineffective.
  • Novel drug-eluting stents (DES) have been developed to locally deliver drugs to inhibit ISR.

Purpose of the Study:

  • To discuss the mechanism of action and pharmacokinetics of sirolimus in DES.
  • To evaluate the efficacy and cost-effectiveness of DES compared to bare-metal stents.

Main Methods:

  • Review of mechanism of action and pharmacokinetics of sirolimus.
  • Analysis of clinical trial data on DES efficacy and cost-effectiveness versus bare-metal stents.

Main Results:

Related Experiment Videos

  • Sirolimus-eluting stents (DES) inhibit vascular smooth-muscle-cell proliferation.
  • Four major clinical trials show sirolimus-eluting stents are safe and effective in preventing restenosis in de novo coronary artery lesions.
  • Sirolimus-eluting stents are associated with less ISR compared to non-drug-eluting stents.

Conclusions:

  • Sirolimus-eluting coronary stents reduce in-stent restenosis.
  • Further investigation is needed to establish the definitive role of sirolimus-eluting stents in treating coronary artery occlusion.
