Summary
This case study shows microsurgical removal of a trigeminal arteriovenous malformation (AVM) is possible. However, potential complications suggest stereotactic radiosurgery might be a better option for trigeminal AVMs.

- A 57-year-old man presented with SAH due to a trigeminal AVM.
- Angiography revealed a doubled superior cerebellar artery supplying the AVM.
- Intraoperative findings showed a close relationship between arterial feeders and the trigeminal nerve rootlets.
Findings:
- Complete microsurgical resection of the trigeminal AVM was successfully performed.
- Postoperative complications included subdural hygroma requiring drainage and shunting.
Implications:
- Microsurgical treatment for trigeminal AVMs is technically achievable.
- Stereotactic radiosurgery may offer a less invasive alternative with potentially fewer complications for trigeminal AVMs.
