Background:

  • Multiple system atrophy (MSA) is a progressive neurodegenerative disorder.
  • Awake bruxism is an uncommon condition characterized by involuntary teeth grinding while awake.

Observation:

  • A 51-year-old male patient with diagnosed multiple system atrophy presented with new-onset awake bruxism.
  • Electromyographic (EMG) studies indicated significant side-to-side amplitude asymmetry in the motor activity bursts associated with bruxism.

Findings:

  • The patient's awake bruxism demonstrated asymmetric motor activity on EMG.
  • Treatment with a low dose of levodopa-carbidopa resulted in a favorable response, alleviating the bruxism symptoms.

Implications:

  • This case suggests a potential link between multiple system atrophy and awake bruxism.
