    Summary

This study on ureterocele with urolithiasis found that surgical approach depends on stone size and patient factors. Successful stone elimination was achieved in 81.6% of patients, increasing to 87.7% with further lithotripsy.

    Area of Science:

    • Urology
    • Surgical Innovation
    • Nephrology

    Context:

    • Ureterocele, a congenital anomaly, frequently coexists with urolithiasis, posing complex treatment challenges.
    • This study focuses on 51 adult patients with true ureterocele and kidney stones, examining various surgical interventions.
    • The mean patient age was 41.9 years, with ureterocele size averaging 20.7 mm and stones 1.4 cm².

    Purpose:

    • To evaluate the efficacy of different surgical strategies for ureterocele complicated by urolithiasis.
    • To determine the optimal surgical approach based on ureterocele characteristics and patient factors.
    • To assess stone-free rates following surgical management.

    Summary:

    • Surgical interventions included ureterocele wall dissection (n=38) and perforation (n=5). Endoscopic procedures were favored for smaller ureteroceles (<30 mm) with preserved ureteral function.
    • Specific endoscopic techniques involved ureterolithoextraction (n=26) and laser/ultrasound lithotripsy (n=19). Open surgery (ureterocystoneostomy) was reserved for larger ureteroceles (>30 mm) or significant upper tract obstruction (n=6).
    • Initial stone elimination was 81.6%, increasing to 87.7% after incorporating extracorporeal shock wave lithotripsy for residual nephroliths.

    Impact:

    • The findings provide crucial guidance for selecting appropriate surgical management in complex ureterocele and urolithiasis cases.
    • Tailoring treatment based on ureterocele size, stone burden, and patient's urinary tract status optimizes outcomes.
    • This research contributes to improved patient care and stone-free rates in this specific patient population.
