Summary

The HeartSmart cardiac rehabilitation program significantly improved quality of life and lifestyle behaviors for rural patients with cardiovascular disease (CVD). These positive outcomes were sustained for at least six months post-program, demonstrating its effectiveness in secondary prevention.
