    Summary

    This study reports on two cases of seborrheic keratosis with unusual histology. The findings suggest a rare variant of irritated seborrheic keratosis with prominent acantholysis.

    Background:

    • Seborrheic keratosis (SK) is a common benign epidermal tumor.
    • Histologic variants of SK exist, but prominent acantholysis is unusual.

    Observation:

    • Two elderly women presented with abdominal lesions clinically resembling seborrheic keratosis.
    • Histologic examination revealed papillomatous acanthoma with significant upper portion acantholysis and dyskeratotic cells.

    Findings:

    • The lesions displayed features of seborrheic keratosis alongside extensive acantholytic changes.
    • Differentiation from other conditions like warty dyskeratoma and acantholytic squamous cell carcinoma was possible based on combined clinical and histologic assessment.

    Implications:

    • The findings suggest a rare variant of irritated seborrheic keratosis.
    • Understanding this variant is crucial for accurate dermatopathologic diagnosis and avoiding misclassification.
