Background:

Observation:

  • A case of orbital varix rupture occurred during cataract surgery with retrobulbar anesthesia.
  • Increased intraocular and intraorbital pressure, followed by severe proptosis, was observed post-anesthesia and during phacoemulsification.

Findings:

  • Magnetic resonance imaging and color Doppler imaging confirmed orbital hemorrhage and varix.
  • Hemorrhage was attributed to venous congestion from anesthetic agents within the muscle cone.

Implications:

  • Silent orbital varices, though asymptomatic, pose a risk during ocular surgery.
  • Anesthesia selection for ophthalmic procedures should account for the potential presence of silent orbital varices to prevent complications like hemorrhage.
