Summary

Custom knee braces offer statistically significant improvements in restraining anterior tibial translation compared to off-the-shelf options. However, the clinical significance of these small, sub-millimeter differences remains uncertain for functional derotation braces.

Area of Science:

  • Orthopedic biomechanics
  • Biomedical engineering
  • Sports medicine

Background:

  • Functional knee orthoses are used to manage anterior tibial instability.
  • Custom-made and off-the-shelf (OTS) braces are available, but their comparative efficacy is debated.
  • Objective biomechanical testing is needed to quantify differences in performance.

Purpose of the Study:

  • To compare the anterior tibial translation restraining capabilities of custom-made versus off-the-shelf functional knee orthoses.
  • To determine if statistically significant differences translate to clinically relevant improvements.

Main Methods:

  • Utilized a pneumatic mechanical surrogate knee interfaced with a servohydraulic materials testing system.
  • Applied anterior/posterior displacements to a maximum anterior load of 400 N.

Related Experiment Videos

  • Tested custom-made and OTS functional knee orthoses from four manufacturers.
  • Main Results:

    • Custom braces demonstrated statistically significant superiority in restraining anterior displacement (P=.0001 to P=.0005).
    • Pooled data revealed custom braces restrained anterior displacement by a mean of 0.84 mm better than OTS braces (P=.0001).

    Conclusions:

    • Custom-made functional knee orthoses provide superior biomechanical restraint of anterior tibial translation compared to OTS braces.
    • The clinical significance of the observed sub-millimeter differences between custom and OTS braces requires further investigation.
