Summary

Contact lens wear can cause reversible corneal opacities and edema in patients with high hyperopia. Discontinuation of contact lenses led to significant visual acuity recovery and corneal clearing.

Surgical Management and Outcomes of Large High Myopic Macular Holes: Global Macular Hole Multicenter Study 3.

Background:

  • Presents a case of bilateral predescemetic opacities in a 62-year-old healthy female with high hyperopia, previously managed with soft contact lenses.
  • The patient exhibited bilateral keratopathy, corneal edema, diffuse predescemetic avascular haze, and Descemet folds, with an unidentified underlying disorder.

Observation:

  • Contact lenses were discontinued, and the patient was treated with topical steroids and artificial tears.
  • Corneal changes including stromal opacity and edema were monitored.

Findings:

  • One year post-contact lens cessation, visual acuity improved from 0.3/0.1 to 0.8/0.63 in the right and left eyes, respectively.
  • Significant clearing of deep stromal opacities and slight recession of corneal edema were observed.

  • Suggests that long-term contact lens wear may induce reversible corneal changes, mimicking or exacerbating underlying conditions.
  • Highlights the importance of considering contact lens-induced keratopathy in the differential diagnosis of unexplained corneal opacities, especially in cases of posterior polymorphous dystrophy.
