Summary

Left atrial stiffness (KLA) is near zero during left ventricular (LV) filling deceleration time (DT). Therefore, DT accurately reflects only LV stiffness (KLV), validating its use in clinical assessments.

Area of Science:

  • Cardiology
  • Biomedical Engineering
  • Physiology

Background:

  • Left ventricular (LV) filling deceleration time (DT) is influenced by both left atrial (LA) and LV stiffness (KLA + KLV).
  • Accurate assessment of LV stiffness (KLV) using DT relies on KLA being negligible during this phase.
  • Quantifying KLA during DT is crucial for validating non-invasive KLV estimations.

Purpose of the Study:

  • To quantify left atrial stiffness (KLA) during left ventricular (LV) filling deceleration time (DT).
  • To determine if KLA significantly impacts the estimation of LV stiffness (KLV) derived from DT.

Main Methods:

  • Assessed KLV using E wave DT from transesophageal echocardiography in 15 post-cardiopulmonary bypass patients.
  • Constructed LA volume curves using mitral and pulmonary vein flow, combined with end-diastolic LA volume.

Related Experiment Videos

  • Measured simultaneous time-adjusted LA pressure to calculate KLA during the V loop and DT.
  • Main Results:

    • LA volume significantly decreased during DT (7.3 ml/m2, P < 0.001), with minimal pulmonary vein inflow.
    • Mean LA pressure remained stable (10.0 mmHg) and did not change significantly during DT.
    • Calculated KLA was close to zero during DT ([-0.002 to -0.001] mmHg/ml), despite LA volume changes.

    Conclusions:

    • Left atrial stiffness (KLA) is negligible and does not change significantly during left ventricular filling deceleration time (DT).
    • The assumption of KLA being near zero during DT is valid.
    • Doppler-derived deceleration time (DT) accurately reflects left ventricular stiffness (KLV) alone, unaffected by KLA.
