Summary
This study compared 99mTc-DPD with a new rhenium-enhanced 99mTc-EDTMP for bone scanning. While 99mTc-DPD showed a better bone lesion to soft tissue ratio, both tracers provided comparable diagnostic interpretations in patients with malignant bone lesions.
Area of Science:
- Nuclear Medicine
- Radiopharmacology
- Oncology Imaging
Background:
- Bone scanning utilizes radiopharmaceuticals to detect malignant bone lesions, improving diagnostic accuracy.
- Commonly used tracers include methylene-diphosphonate (MDP), dicarboxypropane-diphosphonate (DPD), and ethylenediaminetetramethylene-phosphonate (EDTMP).
- 99mTc-labeled MDP and DPD are standard for diagnosis, while 153Sm-EDTMP is used therapeutically.
Purpose of the Study:
- To compare the diagnostic performance of 99mTc-DPD with a novel rhenium-enhanced (carrier-added) 99mTc-EDTMP preparation.
- To evaluate the uptake ratios of bone lesion to soft tissue (BL/ST) and bone lesion to normal bone (BL/NB).
Main Methods:
- A comparative study involving 10 patients with 20 malignant bone lesions.
- Administration of either 99mTc-DPD or carrier-added 99mTc-EDTMP.
- Quantitative analysis of BL/ST and BL/NB ratios at 3 hours post-injection, alongside visual image interpretation.
Main Results:
- Quantitative analysis revealed a significantly higher BL/ST ratio for 99mTc-DPD compared to carrier-added 99mTc-EDTMP (P<0.05).
- No significant difference was observed in the BL/NB ratios between the two tracers.
- Visual assessment indicated comparable diagnostic interpretation between 99mTc-DPD and carrier-added 99mTc-EDTMP bone scintigraphy.
Conclusions:
- Preliminary findings suggest that carrier addition to 99mTc-EDTMP may enhance bone uptake by altering its complex structure.
- While 99mTc-DPD demonstrated superior BL/ST ratios, both tracers yielded clinically comparable imaging results.
- Further controlled trials are necessary to establish any definitive clinical advantage of carrier-added 99mTc-EDTMP over existing 99mTc-based bone scintigraphy tracers.
