Diffuse Large-Cell Lymphoma in the Elderly: Does a Clinically Indolent Subset Exist?

This study found a tendency for negative correlation between leptin and insulin sensitivity (IS) in diffuse toxic goiter (DTG) patients, especially when body mass index (BMI) is controlled. Leptin also showed a tendency for positive correlation with insulin and thyroid stimulating hormone (TSH).

  • To investigate the correlation between leptin and insulin sensitivity (IS) in patients with diffuse toxic goiter (DTG).
  • To explore the relationship between leptin, insulin levels, and thyroid function markers.

  • Descriptive study involving patients diagnosed with DTG.
  • Assessment of body mass index (BMI), serum leptin, insulin (including area under the curve - AuC), thyroid hormones, thyroid stimulating hormone (TSH), glycemia, and insulin sensitivity (IS) using hyperinsulinemic-euglycemic clamp and HOMA-IR.

    • No significant correlation was found between leptin and BMI, thyroid hormones, or glycemia.
    • A significant positive correlation was observed between leptin and insulin levels (r = 0.93, p = 0.001) and between leptin and TSH (r = -0.77, p = 0.042) after controlling for BMI.
    • A tendency for a negative correlation between leptin and IS (HOMA-IR: r = 0.70; PHE: r = -0.55) was noted when BMI effects were considered.

    Conclusions:

    • Leptin levels tend to correlate positively with insulin and TSH in DTG patients, independent of thyroid status.
    • A tendency for negative correlation between leptin and insulin sensitivity exists when BMI is controlled.
    • These findings suggest a complex interplay between leptin, insulin resistance, and thyroid function in DTG.
