Baseline staging for malignant melanoma (MM) is routine but its accuracy is unclear. Sentinel lymph node biopsy is crucial, detecting microscopic disease in most patients, while imaging has limited value for distant metastases.

  • Routine baseline staging for primary cutaneous malignant melanoma (MM) lacks clear diagnostic accuracy data.

Purpose of the Study:

  • To evaluate the sensitivity and specificity of baseline staging methods for detecting regional lymph node and distant metastases in MM patients.
  • To assess the diagnostic performance of physical examination, ultrasound (US), chest X-ray, and positron emission tomography (PET) in MM staging.

Main Methods:

  • One hundred consecutive MM patients with tumor thickness >1.0 mm underwent extensive staging.
  • Methods included physical examination, US of abdomen and lymph nodes, chest X-ray, whole-body PET, and sentinel lymph node biopsy (SLNB).

    • Physical examination combined with lymph node US effectively detects macroscopic lymph node metastases.
    • SLNB identifies a significant number of patients with microscopic disease, often missed by imaging.
    • Whole-body staging at baseline has limited value for detecting distant metastases in MM.
    • Prospective trials are needed to establish the survival benefit of baseline staging and surveillance in cutaneous MM.
