Summary

Active chest compression-decompression cardiopulmonary resuscitation (ACDR CPR) offers no clear survival benefit compared to standard CPR. While not increasing mortality or major complications, ACDR CPR may lead to more skin trauma.

Background:

  • Active compression-decompression cardiopulmonary resuscitation (ACDR CPR) utilizes a suction device for chest compression and active decompression.
  • Previous randomized controlled trials on ACDR CPR have yielded conflicting results.

Purpose of the Study:

  • To compare the clinical effectiveness and safety of ACDR CPR against standard manual cardiopulmonary resuscitation (STR).

Main Methods:

  • A systematic review and meta-analysis of randomized and quasi-randomized controlled trials comparing ACDR CPR with STR in adult cardiac arrest patients.
  • Searches were conducted in major databases (Cochrane Central Register, MEDLINE, EMBASE) up to January 2004.
  • Data extraction and analysis were performed independently, with intention-to-treat principles applied and subgroup analyses by setting and team composition.

Conclusions:

  • Active chest compression-decompression cardiopulmonary resuscitation (ACDR CPR) does not demonstrate a clear clinical benefit in patients with cardiac arrest.
  • The use of ACDR CPR is not associated with increased mortality or major physical complications compared to standard CPR, but may cause more superficial injuries.
