Summary

Endoscopic third ventriculostomy (ETV) is a safe hydrocephalus treatment. However, this case shows that overdrainage can lead to chronic subdural hematoma (CSDH) after ETV, requiring surgical intervention.

Area of Science:

  • Neurosurgery
  • Neurology
  • Medical Imaging

Background:

  • Endoscopic third ventriculostomy (ETV) is a standard procedure for obstructive hydrocephalus.
  • ETV is generally considered a safe and effective treatment option.

Observation:

  • A 69-year-old male patient developed a chronic subdural hematoma (CSDH) four weeks after an ETV procedure.
  • The patient was asymptomatic prior to the development of CSDH.
  • The CSDH was detected via MRI and required surgical evacuation.

Findings:

  • This case suggests that overdrainage, a known complication of shunting procedures, can also occur after ETV.
  • Overdrainage following ETV may be a potential cause for the development of subdural collections like CSDH.
  • Literature review indicates potential causes for CSDH post-ETV.

Related Experiment Videos

Implications:

  • Clinicians should consider the possibility of overdrainage and subsequent CSDH in patients undergoing ETV.
  • Further research may be warranted to elucidate the mechanisms and incidence of CSDH after ETV.
  • This finding highlights the importance of vigilant monitoring for complications after ETV.
