Purpose of the Study:

  • To assess changes in physical function after hip fracture in community-dwelling older adults.
  • To identify baseline factors predicting functional outcomes post-hip fracture.

Main Methods:

  • Prospective cohort study of 120 community-living elders (age 65+) who sustained hip fractures.
  • Self-reported physical function (dressing, transferring, walking, stairs, 0.5-mile walk) assessed pre-fracture, 6 weeks, and 6 months post-fracture.
  • Baseline factors including physical and mental status were evaluated prior to fracture.

Main Results:

  • A sustained decline in physical function was observed at 6 weeks and 6 months post-fracture among survivors.
  • Independent function decreased significantly: dressing (86% to 49%), transferring (90% to 32%), walking across room (75% to 15%), climbing stairs (63% to 8%), walking 0.5 mile (41% to 6%).
  • Pre-fracture physical function and mental status were significant predictors of 6-month functional status.

Conclusions:

  • Hip fracture leads to a substantial and persistent decline in physical function in community-dwelling elderly individuals.
  • Pre-fracture physical and mental health status are critical determinants of recovery and long-term function after hip fracture.
