Background:

  • Thyroidectomy (TX) is an alternative for managing hyperthyroid Graves' disease.
  • It is indicated for patients noncompliant with antithyroid drugs, those with severe ophthalmopathy, large goiters, or who refuse radioactive iodine therapy.

Purpose of the Study:

  • To evaluate the efficacy of rapid preparation for thyroidectomy in severely thyrotoxic patients.
  • To assess the use of iopanoic acid (IOP) and dexamethasone (DEX) in conjunction with other medications for preoperative normalization of thyroid function.

Main Methods:

  • Seventeen severely thyrotoxic patients underwent rapid preparation for TX.
  • Treatment included potent deiodination inhibitors iopanoic acid (IOP) and dexamethasone (DEX), alongside antithyroid drugs (propylthiouracil or methimazole) and beta-blockers.

  • Thyroid function tests (T4, free T4 index, total T3) were monitored before and after treatment.
  • Main Results:

    • IOP and DEX significantly reduced T3 levels (P < 0.0001) and T4/FTI (P < 0.05) in thyrotoxic patients.

    • No postoperative complications such as hypoparathyroidism or nerve damage were reported.

    Conclusions:

    • Rapid preoperative normalization of thyroid function in hyperthyroid Graves' disease is achievable using IOP, DEX, beta-blockers, and antithyroid drugs.
    • This approach is particularly beneficial for patients with large goiters or those requiring urgent surgical intervention.
