Background:

  • Stafne's bone cavities are rare mandibular bone depressions, typically associated with salivary glands.
  • The rarest variants occur on the lingual/buccal aspect of the mandibular ramus, with limited literature documenting such cases.

Observation:

  • A 14-year-old male presented with a mandibular expansion.
  • Radiographic imaging revealed a simple bone cyst and a distinct lesion on the right mandibular ramus.
  • Computed tomography (CT) confirmed a cortical bone defect diagnosed as a mandibular ramus-related Stafne's bone cavity.

Findings:

  • The mandibular ramus lesion, in a young patient, suggests a congenital origin due to ossification failure, rather than a developmental cause.
  • This case challenges the exclusive association of Stafne's bone cavities with salivary glands.

Implications:

  • Mandibular bone depressions may arise from congenital factors, expanding differential diagnoses beyond salivary gland involvement.
  • Further research into the etiology of these rare mandibular defects is warranted.
