Summary

Comparing airway hyperresponsiveness tests in asthma, histamine and exercise challenges showed a significant correlation. Different tests may be needed to fully assess individual patient responses, but histamine is suitable for routine use.

Purpose of the Study:

  • To investigate the correlation between four common bronchial challenge tests for assessing airway hyperresponsiveness in asthma patients.
  • To determine the most appropriate test for routine clinical use.

Main Methods:

  • Ten asthma patients underwent bronchial challenges using histamine, ultrasonically nebulized distilled water (UNDW), exercise, and a relevant allergen.
  • All tests were conducted within a four-week period for each patient, with at least 48 hours between challenges.

  • A significant negative correlation (r = -0.74) was observed between histamine and exercise challenge tests.

  • The findings suggest different underlying mechanisms for bronchoconstriction induced by each challenge.

Conclusions:

  • The assessment of bronchial hyperresponsiveness patterns in individual asthma patients may necessitate the use of multiple challenge tests.
  • Histamine challenge appears to be the most appropriate and practical option for routine clinical assessment of airway hyperresponsiveness in asthma.
