    Area of Science:

    • Thoracic Surgery
    • Vascular Surgery
    • Minimally Invasive Procedures

    Background:

    • Endoscopic thoracic sympathectomy (ETS) is a surgical technique with diverse applications.
    • Long-term outcomes of ETS for various conditions are not always well-documented.

    Purpose of the Study:

    • To present the 30-year results of ETS in a large patient cohort.
    • To evaluate the safety and efficacy of ETS for upper limb arterial occlusion, Raynaud's disease, causalgic pain, hyperhidrosis, and other indications.

    Main Methods:

    • Retrospective analysis of 787 patients undergoing ETS over 30 years.
    • Categorization of patients based on primary indication for surgery.
    • Assessment of immediate and long-term outcomes, including complication rates.

    Main Results:

    • Immediate positive effects were observed in most cases, except for acute arterial thrombosis.
    • Overall complication rates were low (0.2% for lung border damage and hemorrhage).
    • Long-term follow-up (5-10 years) revealed stable positive outcomes in 72.7% of patients.

    Conclusions:

    • ETS is a safe, minimally invasive, and effective intervention for specific conditions.
    • It demonstrates significant long-term efficacy for upper limb chronic arterial insufficiency and hyperhidrosis.
