Area of Science:

  • Neuro-oncology
  • Ophthalmology
  • Pediatric Neurology

Background:

  • Germinomas are rare tumors, typically affecting the central nervous system.
  • Optic nerve involvement by germinomas is infrequent, usually occurring with other intracranial sites.
  • Isolated intracranial optic nerve germinomas represent an extremely rare presentation.

Observation:

  • An 11-year-old boy presented with a three-month history of decreased vision in his left eye.
  • Ophthalmic examination revealed light perception vision only in the affected eye.
  • Magnetic resonance imaging showed a thickened left intracranial optic nerve.

Findings:

  • Surgical exploration via pterional craniotomy revealed diffuse thickening of the left optic nerve.
  • Histopathological analysis of a mass along the optic nerve confirmed it to be a germinoma.

Related Experiment Videos

  • This case represents the first reported instance of exclusive primary intracranial optic nerve involvement by a germinoma.
  • Implications:

    • Highlights the importance of considering rare diagnoses in pediatric vision loss.
    • Adds to the understanding of germinoma presentation and behavior.
    • Suggests the need for specific diagnostic and treatment considerations for isolated optic nerve germinomas.
