Transjugular intrahepatic portosystemic shunts (TIPS) can rarely migrate to the heart, causing serious complications. This case details successful management through combined heart surgery and liver transplantation.

  • This migration led to perforation of the atrial septum.

  • Transvenous removal of the migrated TIPS stent was performed.
  • Attempts at transvenous closure of the atrial septal perforation were unsuccessful.
  • The patient required combined open-heart surgery for septum repair and liver transplantation.
