Summary

A traffic accident caused persistent, painless erections (priapism) in a 48-year-old man. Embolization successfully treated the high-flow priapism, restoring normal erectile function.

Background:

  • Priapism, a prolonged erection, can result from trauma.
  • High-flow priapism is often associated with arterial injury and fistula formation.

Observation:

  • A 48-year-old man presented with a 6-day history of painless priapism following perineal trauma from a traffic accident.
  • Penile blood gas analysis suggested high-flow priapism.

Findings:

  • Selective internal pudendal arteriography revealed an arterial blush in the cavernous arteries, indicative of a fistula.
  • Initial embolization with an autologous clot failed; repeat embolization with Gelfoam successfully occluded the cavernous fistula.

Implications:

  • This case demonstrates successful management of post-traumatic high-flow priapism via embolization.
