Early management involved antacids and histamine 2 receptor antagonists, with limited impact.

Observation:

The relationship between hiatus hernia and reflux is established.
Surgical interventions like open fundoplication yielded modest results compared to pharmacotherapy.

Findings:

Proton pump inhibitors (PPIs) represent a revolutionary advancement in GERD treatment.
PPIs are the most effective current therapy, with comparable efficacy on a milligram-per-milligram basis.

Implications:

Future research could focus on developing acid-suppressive agents with faster onset and longer duration than current PPIs.
