Summary

A rare plexiform fibrohistiocytic tumor was found in a 6-year-old boy. This slow-growing, painless nodule showed no recurrence after surgical removal.

Background:

  • Plexiform fibrohistiocytic tumor (PFHT) is a rare soft tissue neoplasm.
  • This case presents a unique pediatric instance of PFHT in the perioral region.

Observation:

  • A 6-year-old boy presented with a 5mm, hard, erythematous, slow-growing, painless nodule on his right perioral area.
  • Clinical examination revealed a solitary lesion with no history of trauma.
  • Histopathological analysis showed a well-circumscribed, biphasic lesion located intradermally.

Findings:

  • Immunohistochemistry confirmed CD68 positivity in mononuclear macrophages.
  • The tumor was negative for S-100 protein, lysozyme, CD57, and factor XIIIa.
  • Absence of osteoclast-like giant cells was noted, distinguishing it from other giant cell-rich lesions.

Implications:

  • This case expands the clinical and pathological understanding of pediatric PFHT.
  • The intradermal location and lack of giant cells offer valuable diagnostic insights.
  • Successful surgical resection without recurrence highlights favorable prognosis for localized PFHT.
