Background:

Osteosarcoma is a primary bone malignancy.

Observation:

An 18-year-old female presented with multifocal synchronous osteosarcoma.
Lesions were identified in long bones, pelvis, and spine, with the ilium as the dominant site.
Extensive osseous pleural plaque and metastases to lungs, peritoneum, abdominal wall, adrenal gland, and lymph nodes were noted.

Findings:

The patient exhibited widespread skeletal tumor foci.
The extensive osseous pleural plaque suggests advanced metastatic osteosarcoma.
Literature review indicates multifocal osteosarcoma is likely metastatic, not multiple primary sites.

Implications:

This case underscores the aggressive metastatic potential of osteosarcoma.
The presence of osseous pleural plaques warrants further investigation in osteosarcoma patients.
Understanding multifocal osteosarcoma patterns is crucial for diagnosis and treatment strategies.
