Summary

Patients with panic disorder do not necessarily hyperventilate during panic attacks. Symptom severity in panic disorder is linked more to anxiety levels and catastrophic interpretation than respiratory changes.

Exhaled Breath Profiles Before, During and After Exacerbation of COPD: A Prospective Follow-Up Study.

COPD·2019

Area of Science:

  • Psychiatry
  • Psychology
  • Clinical Psychology

Background:

  • The hyperventilation provocation test (HVPT) is a common tool used to study panic disorder.
  • The classical model suggests hyperventilation is central to panic attacks.

Purpose of the Study:

  • To investigate the role of hyperventilation in panic disorder.
  • To compare respiratory physiology and symptom reporting between patients who recognize HVPT-induced symptoms and those who do not.

Main Methods:

  • Forty-eight patients with DSM-III-R Panic Disorder underwent HVPT.
  • Respiratory physiology, panic, hyperventilation, and anxiety symptoms were assessed.
  • Agoraphobia and depression scores were compared between panickers and non-panickers during HVPT.

Related Experiment Videos

Main Results:

  • No significant differences in respiratory physiology were found between recognizers and non-recognizers.
  • Both groups reported comparable panic and anxiety symptoms during daily panic attacks.
  • Panic attacks during HVPT occurred independently of CO2 alterations; higher agoraphobia and depression scores were noted in panickers.

Conclusions:

  • HVPT-induced symptoms in panic disorder are not solely explained by hyperventilation.
  • Symptom severity and panic perception are more strongly associated with anxiety levels and cognitive interpretation.
  • Findings support the cognitive model of panic, emphasizing catastrophic interpretation of somatic sensations.
