Background:

  • Acute interstitial nephritis (AIN) is a kidney disease often caused by medications.

Observation:

  • A 35-year-old woman developed acute renal failure with decreased urine output after 15 days of cefoperazone therapy.
  • Renal biopsy revealed inflammatory infiltrate in the interstitium, supporting AIN diagnosis.
  • The patient recovered renal function after discontinuing cefoperazone and initiating corticosteroid treatment.

Findings:

  • This case represents the first reported instance of cefoperazone-induced acute interstitial nephritis.
  • Diagnosis was confirmed by clinical presentation, urinalysis, eosinophilia, renal biopsy, and response to corticosteroids.
