Background:

  • Pauci-immune glomerulonephritis (PIGN) is a severe kidney disease characterized by inflammation and damage to the glomeruli.
  • Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis can present with rapidly progressive glomerulonephritis, often leading to kidney failure.

Observation:

  • A 68-year-old male presented with macroscopic hematuria, fever, weight loss, and acute kidney injury (creatinine elevated from 1.4 to 4 mg/dL).
  • Renal biopsy revealed segmental glomerular necrosis and crescent formation in 80% of glomeruli, with positive c-ANCA anti-PR3 antibodies.
  • The patient required one dialysis session due to severely impaired renal function.

Findings:

  • Treatment with corticosteroids, cyclophosphamide, and seven plasma exchanges led to rapid improvement in renal function (creatinine decreased from 7 to 3 mg/dL within three weeks).
