Area of Science:

  • Urology
  • Surgical Technology
  • Oncology

Background:

  • Nerve-sparing retroperitoneal lymphadenectomy (RPL) is crucial for preserving ejaculation in testicular cancer patients.
  • Identifying and preserving sympathetic nerves during RPL can be challenging.
  • Hydro-Jet technology offers a novel method for precise surgical dissection.

Purpose of the Study:

  • To evaluate the utility of Hydro-Jet technology for nerve-sparing retroperitoneal lymphadenectomy (RPL).
  • To assess the safety and efficacy of Hydro-Jet dissection in preserving neural and vascular structures.
  • To investigate the application of Hydro-Jet technology in both porcine models and human subjects.

Main Methods:

  • Procedures utilized the Helix Hydro-Jet device with a high-pressure water stream for soft-tissue dissection.

Related Experiment Videos

  • Experimental studies in pigs used pressures of 360-400 psi.
  • Human subjects (n=5) with testicular cancer underwent RPL using pressures of 255-300 psi.
  • Main Results:

    • Hydro-Jet dissection demonstrated significant tissue selectivity, preserving vascular structures and sympathetic nerves.
    • Nerve fibers were resistant to the water jet pressure, allowing for individual isolation.
    • Dissection around great vessels was safe, with no observed injury from direct jet application. Estimated blood loss was minimal to moderate.

    Conclusions:

    • Hydro-Jet dissection is a tissue-selective technique for nerve-sparing RPL.
    • The technology effectively preserves vital vascular and neural structures at pressures of 255-300 psi in humans.
    • Initial experience suggests Hydro-Jet technology is a promising tool for nerve-sparing RPL in testicular cancer treatment.
