Summary

Pyoderma gangrenosum, a severe skin condition, can manifest in patients with inflammatory bowel disease. Infliximab demonstrated significant efficacy in healing these refractory skin lesions.

Background:

  • Pyoderma gangrenosum (PG) is an inflammatory skin condition often associated with inflammatory bowel disease (IBD).
  • Managing extraintestinal manifestations like PG in IBD can be challenging, especially when standard treatments fail.

Observation:

  • A patient with clinically inactive ulcerative colitis developed progressive, necrotic tibial lesions consistent with pyoderma gangrenosum.
  • Conventional treatments including steroids and azathioprine were ineffective in controlling the patient's skin lesions.

Findings:

  • A single dose of infliximab (5 mg/kg) led to a remarkable improvement in the pyoderma gangrenosum lesions.
  • Complete healing of the skin lesions was observed within 3 months of infliximab administration.
