    Area of Science:

    • Psychiatry
    • Neuroscience
    • Pharmacology

    Background:

    • Atypical antipsychotics are commonly prescribed for various psychiatric conditions.
    • Clozapine and olanzapine are frequently used atypical antipsychotics.
    • Obsessive-compulsive (OC) symptoms are a recognized potential side effect.

    Observation:

    • Numerous case reports document OC symptoms with atypical antipsychotics, particularly clozapine.
    • Recent reports detail olanzapine's role in de novo emergence or exacerbation of OC symptoms.
    • Nine cases across six reports specifically examined olanzapine's impact on OC symptoms.

    Findings:

    • Olanzapine was implicated in causing new-onset (de novo) or worsening of existing OC symptoms.
    • In six of the nine reported cases, olanzapine exacerbated pre-existing OC symptoms.
    • In three of the nine cases, olanzapine led to the de novo emergence of OC symptoms.

    Implications:

    • Clinicians should monitor for OC symptoms in patients treated with olanzapine.
    • Further research is needed to understand the mechanisms linking olanzapine to OC symptoms.
    • Risk-benefit assessment is crucial when prescribing olanzapine, considering potential psychiatric side effects.
