Summary

Basal cell carcinoma (BCC) with sebaceous differentiation is rare, but this case confirms its distinct histopathologic features. This finding helps differentiate it from sebaceoma and sebaceous carcinoma.

Background:

  • Sebaceous epithelioma is sometimes used interchangeably with basal cell carcinoma (BCC) exhibiting sebaceous differentiation.
  • However, well-documented cases of BCC with sebaceous differentiation are infrequent in the literature.

Observation:

  • A 72-year-old female presented with a pigmented nodular lesion on the right ala nasi, clinically suspected as BCC.
  • Histopathological examination revealed a BCC with unique features, including duct-like structures and vacuolated cells positive for epithelial membrane antigen (EMA).
  • Apocrine differentiation was also noted within the BCC.

Findings:

  • The presented case confirms the existence of basal cell carcinoma with sebaceous differentiation.
  • Histopathologic findings clearly distinguish this entity from sebaceoma and sebaceous carcinoma.
