Summary

Defining soft-tissue tension in total knee arthroplasty (TKA) remains unclear. This study measured distraction forces during TKA, finding mean forces of 126N in extension and 121N in flexion when proper tension was perceived.

Area of Science:

  • Orthopedic Surgery
  • Biomechanical Engineering
  • Arthroplasty Research

Background:

  • Establishing optimal soft-tissue tension is critical for successful total knee arthroplasty (TKA).
  • Current methods for assessing soft-tissue tension during TKA lack objective, quantifiable measures.
  • The relationship between perceived tension and objective force measurements in TKA requires further investigation.

Purpose of the Study:

  • To objectively measure soft-tissue tension using distraction forces during total knee arthroplasty (TKA).
  • To investigate the correlation between perceived soft-tissue tension and measured distraction forces at specific joint angles.
  • To explore the relationship between intraoperative soft-tissue tension and postoperative range of motion in TKA patients.

Main Methods:

  • A torque driver was attached to a Monogram balancer/tensor device for soft-tissue tension measurement.
  • Distraction forces were recorded in full extension and at 90 degrees of flexion during TKA procedures.
  • Surgeons' subjective assessment of 'proper' soft-tissue tension was correlated with measured distraction forces.

Main Results:

  • The mean distraction force measured when surgeons perceived 'proper' soft-tissue tension was 126N in extension.
  • The mean distraction force measured when surgeons perceived 'proper' soft-tissue tension was 121N in 90 degrees flexion.
  • No significant correlation was found between the measured soft-tissue tension and the final achieved postoperative flexion angle.

Conclusions:

  • This study provides the first objective assessment of distraction forces associated with perceived soft-tissue tension in TKA.
  • The findings suggest that subjective assessment of soft-tissue tension during TKA may not directly predict postoperative flexion.
  • Further research is needed to establish definitive force parameters for optimal soft-tissue tensioning across diverse TKA scenarios.
