Background:

  • Meniere's disease is a complex inner ear disorder.
  • Potential immunologic factors in Meniere's disease remain under investigation.

Observation:

  • Immune parameters including erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), immunoglobulins (IgG, IgM, IgA), complement (C3, C4, CH50), and autoantibodies were assessed.
  • Severe immunologic abnormalities were identified in 16% of bilateral and 2% of unilateral Meniere's disease cases.

Findings:

  • Patients with severe immunologic abnormalities presented with bilateral severe hearing loss and decreased vestibular response.
  • These patients demonstrated excellent responsiveness to steroid treatment.

Implications:

  • Immunologic involvement may play a significant role in a subset of Meniere's disease patients, particularly those with bilateral symptoms.
  • Investigating immunologic abnormalities is crucial for understanding Meniere's disease pathogenesis and optimizing treatment strategies.
