Summary

West Nile virus (WNV) central nervous system infections are rare in US children. This case study details a child presenting with poliomyelitis-like symptoms, where WNV was the sole identified pathogen, a presentation not previously linked to WNV in pediatric cases.

Background:

  • West Nile virus (WNV) neurological infections are infrequent in pediatric populations within the United States.
  • Poliomyelitis-like presentations are typically associated with poliovirus infection.

Observation:

  • A pediatric patient presented with acute flaccid paralysis and other symptoms mimicking poliomyelitis.
  • Extensive diagnostic workup was performed to identify the causative agent.

Findings:

  • West Nile virus (WNV) was identified as the sole pathogen in the cerebrospinal fluid and serum of the affected child.
  • The clinical presentation was indistinguishable from paralytic poliomyelitis.

Implications:

  • This case expands the known clinical spectrum of West Nile virus (WNV) neuroinvasive disease in children.
  • Clinicians should consider WNV in the differential diagnosis of acute flaccid paralysis in children, especially in endemic areas.
  • Further research is needed to understand the pathogenesis of WNV-associated poliomyelitis-like illness in pediatric patients.
