Current anterior cruciate ligament (ACL) surgery outcome scales lack rigorous validation and are often too complex for routine use. Development of a simple, validated scale is needed for accurate patient assessment.

Area of Science:

  • Orthopedic Surgery
  • Sports Medicine
  • Rehabilitation Science

Background:

  • Systematic assessment of anterior cruciate ligament (ACL) surgery outcomes is increasingly important.
  • Numerous patient-reported outcome scales exist, but many lack comprehensive psychometric validation.

Purpose of the Study:

  • To systematically review and compare the psychometric properties of frequently used scales for assessing anterior cruciate ligament (ACL) surgery outcomes.
  • To identify limitations in existing scales and guide future scale development.

Main Methods:

  • A comprehensive literature search of MedLine and Cochrane databases was conducted using keywords related to ACL surgery and rating scores.
  • Four distinct scales (Lysholm and Tegner, Cincinnati, IKDC, and Koos) were reviewed based on reported reliability, validity, and responsiveness.

Related Experiment Videos

  • Psychometric properties were critically evaluated to determine the suitability of each scale for clinical application.
  • Main Results:

    • Test-retest reliability was generally good across scales, with the exception of the International Knee Documentation Committee (IKDC) scale.
    • Construct validity could not be definitively established for any of the reviewed scales.
    • Responsiveness was found to be comparable and acceptable for the Lysholm and Tegner, and Cincinnati scales.

    Conclusions:

    • Existing scales for ACL surgery assessment exhibit insufficient psychometric properties and are often overly complex for routine clinical use.
    • There is a clear need for the development and validation of a simpler, more robust outcome assessment scale.
    • Further investigation into the psychometric properties of the latest versions of the IKDC (IKDC 2000) and Cincinnati scales is warranted.
