To cut or stretch?

Henning Gerke, John Baillie

    Summary

Endoscopic papillary balloon dilation (EPBD) is an alternative to endoscopic sphincterotomy (EST) but carries a higher risk of pancreatitis. EPBD may be best for patients with bleeding risks or difficult anatomy for EST.

    Area of Science:

    • Gastroenterology
    • Endoscopic procedures
    • Biliary interventions

    Background:

    • Endoscopic papillary balloon dilation (EPBD) is explored as an alternative to endoscopic sphincterotomy (EST) to preserve biliary sphincter function.
    • Concerns regarding EPBD's safety profile, particularly pancreatitis, have limited its widespread clinical adoption.
    • A meta-analysis compared EPBD and EST in randomized trials to evaluate their efficacy and safety.

    Discussion:

    • While overall complication rates are similar between EPBD and EST (10.5% vs 10.3%), significant differences exist in specific adverse events.
    • EPBD demonstrated a reduction in post-procedure bleeding compared to EST (0% vs 2.0%).
    • Conversely, EPBD was associated with a higher incidence of post-procedure pancreatitis (7.4% vs 4.3%) and required 'rescue' EST in 20% of cases.

    Key Insights:

    • The meta-analysis indicates comparable overall complication rates for EPBD and EST.
    • EPBD offers a potential advantage by reducing bleeding complications.
    • The increased risk of pancreatitis and need for rescue EST with EPBD are critical considerations.

    Outlook:

    • EPBD may be a suitable option for specific patient populations, such as those with coagulopathy or challenging anatomy for EST.
    • Further research is needed to refine EPBD techniques and patient selection criteria.
    • Optimizing the use of EPBD requires careful consideration of its risk-benefit profile compared to EST.
