Summary

A man died from status epilepticus and respiratory failure. Autopsy revealed B-cell lymphoid cells infiltrating blood vessels, indicating a rare B-cell malignancy.

Background:

Observation:

  • A 67-year-old male presented with status epilepticus and expired due to acute respiratory failure.
  • Autopsy revealed widespread intravascular infiltration by lymphoid cells.
  • Histological examination and immunohistochemistry were performed.

Findings:

  • Immunohistochemical studies showed positive staining for leukocyte common antigen (LCA), L26, and CD20.
  • Negative staining for Factor VIII-related antigen and CD5 was observed.
  • These findings confirmed the neoplastic cells were of B-cell lineage.

Implications:

  • This case highlights a rare presentation of B-cell malignancy.
  • Intravascular lymphoid infiltrates can lead to severe systemic complications.
  • Early diagnosis and understanding of such rare conditions are crucial for patient outcomes.
