Summary

Dilatation or occlusion of the gastrocolic trunk (GT) can indicate pancreatic disease. Computed tomography (CT) can visualize the GT, but cannot distinguish between benign and malignant pancreatic conditions.

Background:

  • The gastrocolic trunk (GT) is a key vessel in the pancreatic and portal venous system.
  • Abnormalities of the GT can be associated with pancreatic pathologies and portal venous system issues.

Purpose of the Study:

  • To evaluate the normal and abnormal appearances of the gastrocolic trunk (GT) and its tributaries using computed tomography (CT).
  • To assess the diagnostic utility of CT in identifying GT abnormalities related to pancreatic diseases.

Main Methods:

  • Retrospective review of CT scans, angiograms, and surgical-pathologic records from 21 patients with pancreatic cancer, 15 with chronic pancreatitis, and 30 controls.
  • Analysis of GT diameter and appearance on CT scans with varying section thicknesses (10mm and 5mm).

Main Results:

  • A normal GT was identified in 48% of controls with 10mm sections and 90% with 5mm sections.
  • GT dilation was observed in patients with splenic vein occlusion or portal-superior mesenteric vein confluence (P-SMVC) occlusion/stenosis.
  • GT obliteration was noted in eight patients, associated with P-SMVC occlusion; CT findings could not differentiate benign from malignant pancreatic diseases.

Conclusions:

  • CT is effective in visualizing the gastrocolic trunk and its tributaries.
  • GT abnormalities, including dilation and obliteration, are associated with pancreatic and portal venous conditions.
  • CT findings of GT abnormalities cannot reliably distinguish between benign and malignant pancreatic diseases.
