High bone marrow microvessel density (BM-MVD) at diagnosis is linked to poorer outcomes in acute myeloid leukemia (AML) patients undergoing chemotherapy and bone marrow transplantation (BMT). Lower BM-MVD after BMT correlates with improved remission, suggesting BM-MVD as a prognostic indicator.
Area of Science:
- Hematology
- Oncology
- Transplantation
Background:
- Bone marrow microvessel density (BM-MVD) is elevated in hematologic disorders.
- Increased BM-MVD in acute leukemias is associated with unfavorable prognosis.
Purpose of the Study:
- To investigate the prognostic significance of BM-MVD in acute myeloid leukemia (AML) patients undergoing chemotherapy and allogeneic bone marrow transplantation (BMT).
Main Methods:
- Retrospective analysis of BM-MVD at diagnosis in 31 AML patients.
- Comparison of BM-MVD between AML patients and controls.
- Correlation of BM-MVD with remission rates, survival, and relapse risk.
- Assessment of BM-MVD changes post-BMT.
Main Results:
- AML patients exhibited significantly higher BM-MVD than controls (median 30/mm² vs. 7/mm²).
- Higher BM-MVD at diagnosis was associated with failure to achieve complete remission (CR) (median 41.5/mm² vs. 28.5/mm²).
- Elevated BM-MVD (>30 mm²) correlated with shorter overall survival and increased relapse risk.
- BM-MVD decreased significantly by day 100 post-BMT in patients achieving continuous CR.
Conclusions:
- BM-MVD at diagnosis is a significant prognostic factor for survival in AML patients undergoing allogeneic BMT.
- Decreased BM-MVD post-BMT indicates a positive response to treatment.
- BM-MVD holds potential as a predictive biomarker in AML treatment strategies.
