This case report details an unusual Epstein-Barr virus-positive Natural Killer (NK) cell lymphoma/leukemia with a prolonged course. The rare cancer presented uniquely, showing good treatment response despite atypical features.

Background:

Natural killer (NK) cell lymphomas are uncommon malignancies, with geographical variations in incidence.
Epstein-Barr virus (EBV) is implicated in the pathogenesis of certain NK cell lymphomas.
Pediatric cases of NK cell lymphomas are exceedingly rare.

Observation:

A unique case of EBV-positive NK lymphoma/leukemia is presented.
The patient exhibited a protracted clinical course with a significant delay between initial symptoms and diagnosis.
Neoplastic cells were detected in peripheral blood but absent in bone marrow aspirates.

Findings:

The lymphoma/leukemia demonstrated peculiar diagnostic features, including peripheral blood involvement without bone marrow infiltration.

The condition followed an unusually long clinical trajectory.

Despite atypical presentation, the neoplastic cells responded well to therapeutic interventions.

Implications:

This case highlights the diverse clinical and diagnostic spectrum of EBV-positive NK cell lymphomas.
Understanding these peculiar features is crucial for accurate diagnosis and effective management of rare hematologic malignancies.
Further research into the unique biological behavior of such cases may inform treatment strategies for NK cell lymphomas.
