Hyoid suspension surgery effectively treats obstructive sleep apnea (OSA) by stabilizing airways. This multi-level surgical approach offers an alternative to CPAP therapy, especially for patients intolerant to continuous positive airway pressure.

Background:

  • Obstructive sleep apnea (OSA) involves upper airway collapse during sleep.
  • Nasal continuous positive airway pressure (nCPAP) is the standard OSA treatment, but compliance is limited (60-70%).
  • Multi-level surgeries combining base of tongue and soft palate procedures aim to stabilize the airway.

Purpose of the Study:

  • To investigate the effectiveness of hyoid suspension as part of multi-level OSA surgery.
  • To compare the outcomes of hyoid suspension surgery with CPAP therapy.

Main Methods:

  • 66 OSA patients underwent hyoid suspension combined with base of tongue, soft palate, or nasal surgery.
  • Pre- and postoperative polysomnography and Epworth Sleepiness Scale (ESS) assessments were performed.
  • Mean patient age was 52.1 years, with a mean BMI of 28.04 kg/m².

Main Results:

  • The mean apnea-hypopnea index (AHI) significantly decreased from 38.9/h to 19.2/h (p<0.0001).
  • Statistically significant improvements were observed in arousal index, oxygen saturation, and ESS (p<0.05).
  • 78.8% of patients were considered responders, with 57.6% cured (AHI < 15/h and ≥50% reduction).

Conclusions:

  • Multi-level surgery incorporating hyoid suspension is effective for OSA treatment.
  • This surgical strategy may be a viable alternative to CPAP, particularly for CPAP-intolerant patients.
