Background:

  • Stroke rehabilitation often involves intensive therapy to regain motor function.
  • Forced-Use Therapy (FUT) is a technique aimed at improving upper limb function post-stroke.
  • The efficacy of FUT without additional shaping therapy during the rehabilitation phase requires further investigation.

Purpose of the Study:

  • To evaluate the effectiveness, safety, and compliance of Forced-Use Therapy (FUT) in stroke survivors.
  • To assess FUT's impact on arm and hand function, postural control, and shoulder pain.
  • To determine if FUT, applied without shaping therapy, enhances recovery during the rehabilitation phase.

  • A prospective, randomized controlled trial was conducted at a tertiary rehabilitation center.

  • Thirty stroke patients with minimal arm/hand movement were randomized into a control group (n=13) or FUT group (n=10).
  • FUT involved wearing a constraint mitten on the unaffected arm for up to 6 hours daily.

    • FUT participants demonstrated 20% greater arm recovery and improved postural control compared to controls (P=.04).
    • Men showed the most significant benefit from the FUT intervention.
    • Compliance with FUT was correlated with cognitive status, and a trend towards increased shoulder pain was observed in the FUT group.

    Conclusions:

    • Forced-Use Therapy, when used without shaping, appears to enhance arm recovery in stroke patients.
    • The FUT intervention was found to be safe and well-tolerated.
    • Further research with larger sample sizes is necessary to validate these findings and investigate the link between FUT and hemiplegic shoulder pain.
