Summary

Adrenal cysts can be associated with tumors, including adrenal cortical carcinomas. Thorough pathologic examination of resected adrenal cysts is crucial for detecting malignancy.

Background:

  • Cystic lesions of the adrenal gland are rare, presenting diverse histologic features from benign pseudocysts to malignant neoplasms.
  • Distinguishing benign from malignant cystic adrenal tumors is clinically significant.

Purpose of the Study:

  • To analyze clinicopathologic features and histologic spectrum of primary adrenal cysts.
  • To identify features aiding in differentiating benign from malignant cystic adrenal tumors.

Main Methods:

  • Retrospective analysis of 41 surgically resected primary cystic adrenal lesions over 25 years.
  • Evaluation of clinicopathologic data and histologic findings.

Main Results:

  • 32 pseudocysts, 8 endothelial cysts, and 1 epithelial cyst were identified.
  • Six pseudocysts were associated with adrenal neoplasms (2 adrenal cortical carcinomas, 2 adenomas, 2 pheochromocytomas).
  • One pheochromocytoma was associated with an endothelial cyst; both patients with cystic adrenal cortical carcinomas had fatal outcomes.

Conclusions:

  • Adrenal neoplasms, including carcinomas, can present as benign-appearing cysts.
  • Extensive pathologic sampling is essential for ruling out malignancy in cystic adrenal lesions.
