Area of Science:

  • Nephrology
  • Urology
  • Gastroenterology

Background:

  • Chronic anemia can present with varied and sometimes unexpected findings.
  • Ureteral calculi (kidney stones) can lead to significant renal complications if left untreated.

Observation:

  • A 38-year-old female patient with chronic anemia was incidentally found to have a nonfunctioning left kidney.
  • Excretory urography revealed multiple asymptomatic ureteral calculi as the cause of renal dysfunction.
  • The patient had a history of self-limited left flank pain approximately 10 years prior.

Findings:

  • Antegrade pyelography and computerized tomography (CT) were performed via a nephrostomy tube.
  • These investigations identified a pyonephrotic renogastric fistula on the left side.
  • This indicates a severe infection within the kidney communicating with the stomach.

Implications:

  • This case highlights the potential for severe, long-term complications from asymptomatic ureteral calculi.
  • Renogastric fistulas are rare but serious conditions requiring prompt diagnosis and management.
  • Early detection and treatment of kidney stones are crucial to prevent such rare but devastating outcomes.
