Background:

Brucellosis is a zoonotic infection with diverse clinical manifestations.
Ocular involvement in brucellosis is rare but can be severe.

Observation:

A young adult presented with rapid bilateral vision loss.
Ophthalmic examination revealed optic disc swelling, optic neuropathy, and serous retinal detachment.

Findings:

The patient had confirmed chronic active brucellosis with high serum antibody titers (1:640).
Brucella antibodies were positive, indicating a direct link to the infection.

Implications:

This case underscores optic neuropathy as a potential cause of irreversible vision impairment in brucellosis patients.
Early recognition and management of brucellosis are crucial to prevent permanent visual deficits.
