Summary

Ibuprofen overdose in adults rarely causes serious toxicity. Renal function tests are not routinely needed for ingestions under 6 grams, and 4 hours of observation suffices for asymptomatic patients.

Area of Science:

  • Toxicology
  • Pharmacokinetics
  • Clinical Medicine

Background:

  • Ibuprofen overdose management guidelines require further clarification.
  • Assessing the risk of renal injury and central nervous system (CNS) toxicity is crucial.

Purpose of the Study:

  • To determine the incidence of renal injury in ibuprofen overdose cases.
  • To evaluate the utility of timed plasma ibuprofen levels in patient management.

Main Methods:

  • Prospective study of 63 adult ibuprofen overdose cases.
  • Analysis of timed plasma ibuprofen levels and comparison with a nomogram.
  • Monitoring for central nervous system (CNS) and renal toxicity.

Main Results:

  • No serious toxicity was observed in any cases.
  • Ingestion of less than 3 grams of ibuprofen did not result in significant CNS or other toxicity.
  • Two patients ingesting 4 and 4.8 grams had minor blood urea nitrogen elevations despite normal serum creatinine.
  • A revised nomogram may aid in predicting outcomes for asymptomatic patients.

Conclusions:

  • Renal function tests are not routinely indicated for ibuprofen ingestions under 6 grams.
  • Four hours of observation is adequate for asymptomatic patients not requiring psychiatric admission.
  • Timed plasma ibuprofen levels are not necessary for effective patient management in overdose cases.
