This case report details an 80-year-old woman with a limbal dermoid, a benign tumor. While typically removed in childhood, this limbal dermoid was excised later in life due to its size and symptoms.

Background:

  • Solid limbal dermoids are usually surgically excised in pre-school aged children.
  • Intervention may be earlier if high irregular astigmatism poses a risk for amblyopia.

Observation:

  • An 80-year-old female presented with a two-month history of left eye irritation, foreign body sensation, and tearing.
  • Ocular examination revealed a prominent, vascularized limbal tumor with secondary stromal lipoid deposits and deep anterior chamber penetration.
  • Histological analysis confirmed a chronically irritated, predescemetal limbal dermoid with epidermalization and degenerative changes.

Findings:

  • The limbal dermoid was excised in an elderly patient, deviating from the typical pre-school age intervention.
  • Histopathology revealed significant secondary changes including epidermalization and elastoid degeneration.

Implications:

  • Surgical excision of limbal dermoids is recommended in pre-school age when medically or cosmetically indicated.
  • Lamellar excision is the preferred surgical approach for limbal dermoids.
