Area of Science:

  • Nephrology
  • Pharmacology
  • Infectious Diseases

Background:

  • Vascular-access-related bacteremia is a common complication in hemodialysis patients.
  • Vancomycin is frequently used to treat these infections, requiring careful therapeutic drug monitoring.
  • Traditional vancomycin dosing algorithms in hemodialysis necessitate frequent serum concentration measurements.

Purpose of the Study:

  • To investigate the feasibility of a limited-sampling vancomycin dosing algorithm.
  • To assess its effectiveness in outpatient high-flux hemodialysis for bacteremia.
  • To evaluate potential cost savings associated with a simplified monitoring approach.

Main Methods:

  • Retrospective analysis of vancomycin concentration data from an established dosing algorithm.

Related Experiment Videos

  • Evaluation of pharmacokinetic parameters and vancomycin removal rates during hemodialysis.
  • Implementation and retrospective analysis of a revised, limited-sampling vancomycin dosing algorithm.
  • Main Results:

    • The original algorithm yielded 97% of vancomycin concentrations within the target range (5-20 microg/mL).
    • Mean vancomycin removal during hemodialysis was 39% +/- 13%.
    • The revised limited-sampling algorithm reduced vancomycin concentration determinations by 70%, with 93% of levels remaining therapeutic, and estimated annual cost savings of $7552.

    Conclusions:

    • A vancomycin dosing algorithm with limited concentration monitoring is feasible for hemodialysis patients.
    • This approach achieves comparable therapeutic vancomycin concentrations to more intensive monitoring.
    • The revised algorithm offers significant cost savings for hemodialysis units.
